This secret woman we are talking about can be found during your Side Ops: Prisoner Extraction 02. As you begin with the mission, you'll notice extremely rare woman prisoners are spawn. Extract her and she has a default codename assigned called the "Obsidian Mole". This is how you'll unlock her.

Once you have successfully completed the mission and unlock her. Now it's time to choose the character you'd like to play during your missions. Select a Mission and after that Go to Character and you can select her as a player model and play the rest of the game with her. She has a very High-Combat stats which affects your health pool.
