Both Ciri and Geralt have several fates that are realized under a specific set of circumstances. It might be related to someone you let live, or a place that you visit during a particular quest. In some cases, it will be a choice that you would never predict to have the ripple effect it will.
Of course, there are major spoilers to follow, so you may want to avoid reading if you're not interested in how the game could potentially end. This is intended for players that want to control how things turn out when all the quests are completed.
Note - this article contains major spoilers!Ciri's fate is decided by a series of choices that you make during three specific quests. If you make more positive choices than negative, she'll end up with a more favorable fate. If you choose more negative than positive, Ciri's future may not be quite as bright.
The five potential choices are found in the following quests.

Choice #1 - During Blood on the Battlefield, reply to Ciri with Think I know what might lift your spirts (+1), or Relax, you don't have to be good at everything (-1). The former will allow you to take part in a snowball fight, cheering Ciri up a bit.
Note - During Blood on the Battlefield, Ciri will want to head into Velen and go after Imlerith. You can choose Gotta visit the emperor first, or All right, Velen it is. Visiting the emperor opens up Choice #2.
Choice #2 - During Father's Day, the emperor will attempt to pay you for bringing Ciri to him. Refuse to accept payment (+1), or choose to accept it (-1). If you refuse payment Ciri will be proud of you, working towards a positive ending.
Choice #3 - During Final Preparations, you can choose to remain on the landing (+1), or accompany Ciri (-1). If you choose to allow Ciri to speak to the sorceresses alone she will gain confidence in herself, helping with a positive ending.
Choice #4 - During Child of the Elder Blood, Ciri wants to ransack Avallac'h's laboratory. Respond with Go for it (+1), or Calm down (-1). The latter option will weaken Ciri's resolve, while the former will only serve to strengthen it.
Choice #5 - Also during Child of the Elder Blood, Ciri will decide she wants to travel to Hindarsfjall to visit the grave of a boy who saved her. You can respond with Yeah, I'll go with you (+1), or Not time (-1). The latter choice weakens Ciri.
This outcome becomes a possibility if you ended up with more negative choices than positive. This will unlock Something Ends, Something Begins I. This doesn't necessarily mean that Ciri is in fact dead, but it is considered the most negative of the three possible endings.
During Blood on the Battlefield, Ciri asked you to go after Imlerith, and you had the option of agreeing, or going to see the emperor first. If you chose to see the emperor, it opened up Something Ends, Something Begins II as a possible ending. The key here is that you still have to have more positive choices than negative, Radovid must be dead, and you must have sided with Roche and Thaler.
This ending is possible if you had more positive choices than negative, teaching Ciri all that you knew as a Witcher. The other key component to achieving this ending is that you must not have gone to see the emperor first during Blood on the Battlefield. This will unlock Something Ends, Something Begins III.
This ending is possible if you complete the secondary quest, Now or Never and tell Triss that you love her and want to be with her. You must also ignore the advances of Yennefer during the secondary quest, Last Wish.
This ending basically works in reverse from the last one. You would need to choose to be with Yennefer during the secondary quest, Last Wish, while also ignoring the advances of Triss in Now or Never.
There are two ways that Geralt can end up living the life of a witcher forever. The first is by ignoring both Yennefer and Triss in the secondary quests, Never Alone and Last Wish. The second is that if you tell them both that you love them and want to be with them. Basically, you have to choose one or the other. If you choose both you'll end up alone. You'll also end up alone if you don't complete either of the two secondary quests.
