If taken this way, it is in the clinic where you speak to Amata. It is in the clinic's back room on a desk.* If you do not take the bobblehead prior to your initial escape from Vault 101,trouble on the homefront is your last chance to get it, as you'll be permanently banned from Vault 101 afterward unless you choose to sabotage the vault.
