Chapter 4: The Patient

Once the chapter begins follow Dr. Jimenez forward and through the small gap in the brick wall. Head through the bloody closed door to your right and interact with the mirror to temporarily return to the hospital and purchase a few upgrades.

Once you’re in the hospital bed leave your room and turn to the right, following the hall through the reception area and the iron gate. Sit in the chair to grab your upgrades and once you’re done cross to the opposite side of the hallway and interact with the mirror once more to return to the chapter.

Exit the small shack and twist to your right, passing through the nearby door and grabbing any loot you see. Dr. Jimenez will spot his brother, Valerio, who appears to be working on a patient. However, upon closer inspection things turn quite ugly. Once you regain control immediately put four rounds from your Handgun into his head.

With your enemy down head into the room where he was conducting his business and interact with the body on the table. It might take a little work but eventually you’ll dig a key out of the patient.

Head upstairs, spending a few moments checking for loot, and then backtrack out of the house. Head up along the street, making a left turn and moving up to where a cart is parked along the left shoulder. Now turn into the walkway just before the cart. Kick the door open and head into the next room, following the stairs down two flights into a hallway. Be sure to dip into the room on the left and grab the supplies before moving forward.

Head through the door at the end of the corridor to find Leslie in the back room. You’ll also be attacked here so go ahead and have your Shotgun ready to take the enemy down. Once the coast is clear backtrack the way you came to trigger a cut scene that ends with Ruvik walking away from you. Do your best to follow him, eventually triggering another cut scene that ends with you sitting in a basement full of blood and guts.

We know… we were scratching our heads a little bit to. Head up onto the catwalk and crawl forward to sneak under the nearby trap. When you approach the door you’ll get another brief look at Ruvik before being attacked by a large batch of enemies. If you lure the enemies through the traps in the area you can take them all down pretty easily. Just wait for the enemies to follow you in, and once they’re close enough, just shoot the barrels to kill several of them at once.

Once you’ve cleared out the uglies head through the now open door that Ruvik was standing at. Spend a few moments looting up in the area, burning any corpses you find, and grabbing the Shock Bolt you’ll spot in the loot. When you’re done head through the door and follow the corridor the only direction you can. Move into the next room and forward until a giant Spider attacks you.

Don’t even bother trying to fight this creature, instead run back down the corridor and hangout by the white door. When the spider gets close the door will open, which is your cue to run like your life depends on it (cause it kind of does). Follow the path until you reach a ladder and then head down the ladder and follow the hallway. Duck under any traps and then sprint into the elevator to catch your breath. The ride isn’t very long and once it’s over you’ll need to start running again.

You guessed it! Our friendly neighborhood giant Spider friend is back so you’ll need to sprint forward and through the door on your left. Keep moving as quickly as you can to dive under the door before it shuts. Once you regain control you’ll be on a set of large metal stairs. Descend them until you see Ruvik and then head back up to trigger the chapter ending cut scene.
