Sounds like a bad disk. Clean it with a soft cloth and check there are no finger prints, marks or dirt on it afterwards.

If that fails take it into a video game store, I know gamestation repair disks for £2 each and there service is very good.

However If your disk has a circle shaped scratch mark that goes all the way around the disk (laser burn) then there is no way to fix it. This is caused by moving your XBOX when the disk is still spinning.
