What specifically causes a fall through floor glitch in video games

The thing that causes this is that the creators of the game(s) had failed to fully put down all of the digital ground in the games, or you may have gotten out specific area in the game that wasn't intended to let you out of and the creators made the ground out so far and didn't continue because it would've been pointless.
